We collect information that is necessary to provide safe, high-quality medical care. This may include:

  • Personal details (name, date of birth, address, phone number, Medicare number, next of kin)

  • Medical history, current health information, medications, allergies, risk factors, and family history

  • Test results, specialist correspondence, hospital discharge summaries, and other clinical information

  • Billing and account information

We may collect information:

  • Directly from you (in person, via phone, online, or in writing)

  • From other health professionals, specialists, hospitals, pharmacies, pathology, or imaging providers (with your consent)

  • From Medicare, My Health Record, or private health funds (where relevant)

Your personal information is used to:

  • Provide medical treatment and health care services

  • Communicate with other health professionals involved in your care

  • Send reminders and recalls for follow-up care, immunisations, or health checks

  • Manage billing, payments, and compliance with Medicare, DVA, and private health funds

  • Conduct practice quality improvement and accreditation activities (de-identified where possible)

  • Meet legal and regulatory requirements

4. Disclosure of information


We may disclose your information to:

  • Other treating health professionals (e.g., specialists, allied health, hospitals, pathology and radiology providers)

  • Medicare, DVA, or private health insurers where required for billing

  • Our IT providers, auditors, and accreditation agencies (under strict confidentiality agreements)

  • Third parties when required by law (e.g., public health reporting, subpoenas, mandatory reporting)

  • We will not disclose your information to any third party without your consent, unless legally required.

 

5. Data storage and security

 

  • All patient records are stored securely in electronic and/or paper format.

  • Access is restricted to authorised staff who require the information for their role.

  • We maintain secure computer systems with passwords, firewalls, and data backups.

  • Staff receive regular training in privacy and confidentiality.

6. Access to and correction of your health information

 

  • You have the right to request access to your personal health record.

  • Requests must be made in writing and may be subject to an administration fee.

  • We may require identification before releasing information.

  • If information is inaccurate, incomplete, or outdated, you may request correction.
